Filing A Complaint

Please use your TAB key to move through the fields.  Using the enter key will submit the form to the HRC.  If you press the enter key, use the back button on your toolbar to return to the questionnaire.

Please answer ALL questions.  An incomplete questionnaire can slow down the process of formally filing a complaint.

Once you have submitted a completed online questionnaire, an Investigator from the HRC will contact you to schedule an appointment to formally file the complaint.  The questionnaire is a means of the HRC for obtaining the necessary information to file a formal complaint, it is NOT a formal complaint.  You must sign an official complaint form before the HRC can investigate your claim.
